Job Title: Care Co-ordinator

To be responsible to the Registered Manager for the day-to-day service provision to the Service Users by producing detailed work schedules/rota for the Care staff and providing both written and verbal instructions/guidance to them. Given the wide remit of the job, a care coordinator’s regular duties can be quite varied. Meeting with the service user, kinship carers, and other family members to carry out an initial assessment and discuss their needs, the services available to them and the help/support they would like. Ensuring that the care staff are compatible with the service user they are going to visit and support. A care plan is drawn up on this basis of assessments and information from other care professionals, the initial assessment and meeting with the service user. The coordinator is then responsible for contacting other care departments or agencies to ensure that services are delivered appropriately.
On call duties pro rota.
Duties & Responsibilities:
To always represent the Company in a professional manner, on the telephone, face to face or in written communication.
To ensure that telephones are answered promptly, and people are spoken to in a polite and respectful manner.
To always maintain confidentiality and carry out the Company’s Confidentiality Policy.
Completing the weekly rota, ensuring calls are covered and where required attending calls, this will ensure that all calls are covered, and there are no missed or cancelled calls.
Developing care plans and rearranging them as necessary when difficulties arise.
To carry out the monitoring of the service in the field to ensure that it meets the quality targets of the Company, contractual requirements of the commissioners and CQC standards, and to keep the Registered Manager informed of the outcomes and issues that are identified.
To visit Service Users and to make reviews of the written Care Plans, examine the written records of medication administration, finance and daily log notes kept by Staff as required by the Registered Manager.
Working with the care team to evaluate interventions and identify where and when further ones will be required.
To assist the Registered Manager with the recruitment, induction, training, and supervision of Staff.
Every 4-6 weeks or when required to carry out spot checks of the Staff’s performance.
Supervision reviews of the Staff’s performance.
To produce reports as required by the Registered Manager.
To maintain accurate records.
To assist the Registered Manager in holding team meetings for Care Workers.
Reading, attending workshops and liaising with professional bodies to stay abreast of developments in the field.
Essential:
Experience in care provision at a level that reflects the responsibilities of the post applied for
Good literacy and numeracy skills
Good presentation skills
Computer literate
Understanding of legislation concerned with care provision
Administrative experience
Full driving licence Desirable:
NVQ 3Professional qualification or equivalent
